MEDICATIONS
Indiana law requires a written order from a physician for a medication to be given at school. A parent/guardian signature is also required. Medication forms may be obtained in the nurse's office or by using the link below. Written permission is also required for over the counter medications given at school.
- Prescription medications must be in the original container with the label issued from the pharmacy. Over the counter medications should be in the original container.
- To give medication at school to your child, a form must be completed and signed by the parent or guardian.
- Prescription Medication Form (over the counter medication is page 2 of the form)
- The school does not stock medications like acetominophen or ibuprofen to administer to students. Each student must have their own bottle delivered to the clinic by a parent or guardian and written permission given to the nurse.
- Cough drops are not permitted at school per BCSC policy due to the potential choking hazard.
- All medications are stored in a locked medicine cabinet in the nurse's office.
