Medications at School
Indiana law requires a written order from a physician for a medication to be given at school. A parent/guardian signature is also required. Medication forms may be obtained in the nurse's office or by using the link below. Written permission is also required for over the counter medications given at school.- Prescription medications must be in the original container with the label issued from the pharmacy. Over the counter medications should be in the original container.
- To give medication at school to your child, a form must be completed and signed by the parent or guardian.

- The school does not stock medications like acetominophen or ibuprofen to administer to students. Each student must have their own bottle delivered to the clinic by a parent or guardian and written permission given to the nurse.
- Cough drops are not permitted at school per BCSC policy due to the potential choking hazard.
- All medications are stored in a locked medicine cabinet in the nurse's office.

Vision
All students in grades 1, 3, 5, and 8. A modified clinical technique is done by volunteer optometrists for grade 1 students.

Helpful Tips - When to keep your child home
- Fever: temperature of 100.4 degrees or more. No return until the temperature is under 100.4 degrees without fever reducing medication for the last 24 hours.
- Vomiting: vomiting 2 or more times in 24 hours
- Diarrhea: 2 or more episodes of diarrhea in the last 24 hours
- Eyes: crusting of eyelids or lashes, purulent drainage from the eye or pink eye (bacterial conjunctivitis). May return 24 hours after beginning antibiotic treatment.
- Skin: unknown rashes or skin lesions (open sores)
Medications:All medication must be brought to school by a parent or guardian in the original container. Prescription medications must also have a doctor's prescription brought or faxed to the nurse. In order to administer any medication, a form must be filled out and signed by the parent or guardian. The school does not stock acetaminophen or ibuprofen to administer to students. Each student must have their own brought in to the clinic by a parent or guardian and written permission given to the nurse.
